I hereby make application for a 48 hour test preparation in C.R.C.S.T and agree to the terms and conditions herein contained and agree to pay Mbawine Consultants $3,500.00 by one of the following plans:

1. ONE TIME PAYMENT: $3,500.00.

2. PAY AS YOU GO: $3,500.00. Minimum of $2,000.00 to start then pay $150.00 weekly or $300.00 biweekly before
conclusion of The Test preparation.

3. FINANCE PAYMENT OPTION: Loans available based on qualification.

1. FINANCE: $3,500.00. With a down payment, in six  months. Finance through a Major Bank with interest for up to six
months. Subject to credit approval. Certain restrictions apply.  If approved, the Credit Covers Test preparation, Books, and Final Master Exam.

APPLICANT AGREES TO ABIDE BY THESE ADDITIONAL TERMS AND CONDITIONS.

1. Regular attendance is required for all Test Preps. Tardiness will be considered an absence. If an applicant is absent due to illness or other good cause, and notifies Mbawine Consultants prior to the start of the scheduled Test Prep, the absence may be excused and a makeup can be arranged for the applicant at no additional cost.

2. Applicants on “the pay as you go” payment plan should note that when a payment is missed, one will not be allowed
into the Test Prep until the balance is made current.

3. No profanity, disorderly conduct or other disruptive action will be accepted. Applicants may be terminated for violations of these standards.

4. Accelerated applicants who make below 80% on both quizzes and progress test, on three conservative times will be moved to the regular program. Applicants in the standard test  Prep, who make below 70% on both quizzes and progress test, on three conservative times, will be held back if their grades do not improve

NOTE: Each applicant, upon successful completion of the program will be assisted by the office managers in job search assistance.

IT IS UNDERSTOOD THAT MBAWINE CONSULTANTS CANNOT PROMISE OR GUARANTEE EMPLOYMENT.

Photo/Video Release:

Mbawine Consultants is hereby granted permission to take photographs and video of the applicants for security reasons, IDs and to use in fliers, websites, social media, advertisements and other promotional materials we create. Permission is also hereby granted for Mbawine Consultants to copyright such photographs in the company name.



MBAWINE REFUND POLICY

● An applicant who cancels within 7 days of signing the enrollment agreement, but before Test Prep begins, receives all
money paid with the exception of the non-refundable fee of two thousand dollars.

● An applicant who withdraws after the seven days will forfeit ALL deposit payments in cashback. A credit in the deposit
amount could be applied as directed by the payee towards future test preps. Or applied towards a chosen applicant’s test
prep fee. This credit will be valid for ONLY 12 months.

● Thereafter, applicant will be liable for:

a. The cost of books plus

b. Test prep liability as of the applicants’ last date of physical attendance, HSPA exam & Clinicals

This constitutes the enrollment agreement between Mbawine Consultants and the applicant, and no promises or agreement are orally or in writing, expressed or implied. The agreement represents the entire understanding and supersedes all prior statements, agreements and understanding, if any, either oral or written.

This is a test prep review designed to prepare graduates to sit for the HSPA certification exams. All training is done in the sterile processing department in a healthcare environment, through a volunteer department, to meet HSPA’s training requirements in order to obtain full   certification from HSPA.
